What is an os acromiale ?
It is a failure of the bony fusion at the front of the shoulder roof ( the acromion ). A mobile bone remains at the anterior acromion. It´s a rare condition.
Signs and symptoms of an os acromiale ?
Shoulder pain and pain in the upper arm are charcteristically. Pain is present while working with the affected arm in or above shoulder heights ( e.g. throwing, cleaning windows ). Os acromiale can be misunderstood or diagnosed as an impingement syndrome.
Development of an os acromiale ?
A gap remains at the anterior acromion due to a non fusion of the bone at the shoulder roof. An os acromiale is inherent.
How to treat an os acromiale ?
If os acromiale is causing pain it has to be treated. There is one problematic aspect: an os acromiale is strongly associated with rotator cuff tears. The mobile bone is causing pressure on the supraspinatus tendon and over years, this leads to a fraying and then to a defect in the rotator cuff.
Depending on it´s size the os acromiale is resected in an arthroscopic operation. if it is small. Sometimes it is removed in an open operation. If it is bigger an osteosynthesis, using screws, is performed.
Not every os acromiale has to be removed by an operation. If it is stable it can be left and monitored.
